This encounter can be triggered if you completed a missable "Rough Neighborhood" side quest - Using a companion mod for Ciri - Ultra Settings -
I came across this random (and kind of secret) quest while on my way to a different one. In this quest, Geralt comes across a corpse on a remote island where he finds a journal, the journal talks about a woman and child with no understanding for the common tongue from far and distant lands, and how they are being hunted down by assassins, Geralt embarks on a journey to find out what happened to the woman and child once and for all. Men can be terribly cruel, especially to those of different races. The witcher had seen humans persecute nonhumans far too often. This time he witnessed a particularly appalling scene - a group of thugs wanting to burn a she-elf alive. Fortunately, the witcher managed to defeat the bandits and save the elf. Unfortunately, the witcher cannot be everywhere at once and not every victim of racial persecution can count on his help... Later in Novigrad Geralt saves her for the second time, though she doesn't show any qratitude. And to a witcher's displeased remark she answers if he expected her to "spread her legs" for him in appreciation. Which might have actually happened if it were The Witcher 1. PS.
